The Name of Current ISP internet circuit currently installed at Name of Community has proven inadequate over the past several weeks. Despite our repeated efforts working with Name of Current ISP to resolve the issue the circuit continues to be an issue.

In order to get adequate internet access at this location, our recommendation is to install a new Internet circuit . The monthly recurring cost will be Place Recurring Monthly Fee Here. This would replace the current Old Place Recurring Monthly Fee Here  with the new speed of _______________ .

We need an “Approval” from the RM or DM to make changes of this nature.
